f(x+h)=(x+h)^2+4(x+h)=x^2+2xh+h^2+4x+4h. (I assume the question asks for (f(x+h)-f(x))/h.)

Take away f(x): 2xh+h^2+4h and divide by h: 2x+h+4.

When h is very small the quotient is 2x+4, otherwise h cannot be ignored.
